Local Forms
Local forms are drafted and approved by individual court districts. The local forms on this website are specific to the District of Nevada. If the form you are looking for is not listed below, it may be available as a National form.

Form Categories
Adobe Reader is required to view and print portable document format (PDF) forms. Adobe Acrobat is required to save a PDF fillable form. Click here to download the free Adobe Reader software.
To Save and Upload to ECF
- Open the form needed and fill-in all requested information.
- Select the 'print' option from the menu by clicking the printer icon or going to File>Print.
- In the Printer menu, click the arrow for the Name drop down menu.
- Select 'Adobe PDF' as the printer and click OK.
- The "Save PDF File As" box will pop up. Enter a file name to save the document. Use this file for uploading into ECF.
