Bankruptcy Forms Changes

There are two categories of bankruptcy forms - Official and Local. Forms are available in Portable Document Format (pdf) and/or WordPerfect format (wpd).

If you are unsure which forms to file, check the Filing Requirements page to identify the forms necessary to your bankruptcy type.

Official Forms

Official forms are available for download from the U.S. Courts website and are divided into Official and Procedural forms.

These forms must be used in all court districts and are specified in the Federal Rules of Bankruptcy Procedure.

Local Forms

Local forms are drafted and approved by individual court districts. The local forms on this website are specific to the District of Nevada.
